A healthcare referral system connects health facilities so that patients receive care at the level equipped to meet their needs. It enables transfer for assessment or treatment beyond a facility's capacity, while counter-referral returns patients to the referring facility for continued care when appropriate.
Linking different levels of care
Referral links primary, secondary and tertiary services according to the patient's clinical needs and the facilities' capabilities.
- Primary care provides first-contact assessment, treatment of common conditions and recognition of patients who need specialist or hospital care.
- Secondary care provides specialist assessment and hospital services, receiving patients whose needs exceed primary-care capacity.

Ensuring continuity and safe transfer
Referral is more than directing a patient to another hospital: it requires communication and coordination between the sending and receiving facilities.
- A referral record communicates the clinical findings, treatment already given and reason for referral.
- Appropriate transport and coordination with the receiving facility support safe transfer, especially in emergencies.
- Feedback and counter-referral help the local facility continue medicines, monitoring and follow-up after specialist treatment.
Improving access and resource use
A functioning referral system supports timely access to appropriate expertise while allowing routine care and follow-up to remain closer to home. It promotes efficient resource use by matching patient needs with facility capabilities and reducing unnecessary use of higher-level hospitals.
