RMNCH+A Continuum of Care

RMNCH+A is an integrated approach that provides health services across reproductive life, pregnancy, birth, infancy, childhood and adolescence. Adopted in 2013 and implemented through the National Health Mission, it connects interventions across life stages and service-delivery levels so that beneficiaries receive continuous rather than fragmented care.

Meaning of the continuum

The continuum has two connected dimensions: continuity across time and continuity across places of care. The plus sign highlights the inclusion of adolescence, the linkage of reproductive health and family planning with maternal and child health, and stronger referral links between community and facility-based services.

• The life-course continuum covers adolescence, reproductive age, pregnancy, childbirth, the postnatal period, newborn life and childhood.
• The service-delivery continuum links care at the household, community, outreach and health-facility levels.
• Referral and follow-up connect different levels so that complications can be identified, transferred and managed without interruption.

Main intervention areas

The approach organizes services into five linked components: reproductive, maternal, newborn, child and adolescent health. It combines preventive, promotive, curative and referral services rather than treating each component as a separate programme.

• Reproductive health includes family planning, contraceptive services and prevention and management of reproductive health problems.
• Maternal care includes antenatal care, skilled care during childbirth, emergency obstetric care and postnatal care.
• Newborn and child care includes essential newborn care, immunization, nutrition support and management of common childhood illnesses.
• Adolescent services address nutrition, sexual and reproductive health, mental health, substance misuse, injuries and non-communicable diseases.

Why the approach matters

Health outcomes at one stage influence the next: an adolescent girl's nutrition affects later pregnancy, while maternal health affects newborn survival and development. RMNCH+A therefore promotes integrated planning, early risk detection, timely referral, equitable access and coordinated use of National Health Mission platforms and frontline workers.
